DESCRIPTION:Mount Auburn Hospital presents a free   six week workshop cal
 led &ldquo\;My  Life\, My Health.&rdquo\; This program is  offered  to a
 dults living with\, or  caring for\, someone with a chronic  health  con
 dition. Participants can  learn how to set and meet personal  goals\,  i
 dentify and try out simple  techniques to improve quality of  life\, gai
 n  skills to improve  patient-to-doctor communication\, learn to  manage
   stress\, raise energy  levels\, make educated choices about  treatment
 \, and  have fun!\nThe Chronic Disease Self-Management Program was devel
 oped by    researchers at Stanford University Medical Center and has bee
 n proven to    help people just like you! Participants will receive a co
 py of the    book\, &ldquo\;Living a Healthy Life with Chronic Condition
 s\,&rdquo\; as well as a    relaxation CD.\nThe  program is available on
  November 5th\, 19th\,  and 26th. As well   as  December 3rd and 10th. S
 essions will run from  9:30am-12:00pm and    refreshments will be provid
 ed.
